1. Requesting Your Therapy Records

You have the right to request access to your health care records maintained by this practice, including therapy records, in accordance with applicable state and federal law.
How to request your records:

• Submit a written request by email, mail, or in person.
• Include your full name, date of birth, current contact information, and a clear description of the records requested.
• Identification may be required to protect your confidentiality.

Records will be provided within the timeframes required by Texas law.

1. Licensing and Disciplinary Authority (LPC Oversight)

This practice and its Licensed Professional Counselors are regulated by the Texas Behavioral Health Executive Council (BHEC).
You may contact BHEC if you have concerns regarding professional conduct, ethical violations, or licensing matters.

3. Filing a Consumer Complaint
